Use 'Em or Lose 'Em: Top 5 Reasons to Take Advantage of Your Dental Insurance Benefits Now!

A Holiday Survival Guide for Your Smile

The holidays are upon us, and it is the season for a bit of togetherness (and a lot of indulgence). We don't all celebrate the same holidays or adhere to the same belief systems, but there is one thing that seems to unite us all: comfort food.

Cold weather and sweets go hand-in-hand. What's a good snowball fight without a warm cup of cocoa to come home to? Even in warmer climates, it's difficult to dissuade dreams of sugar plums after a good night of classic movies with your family. So, while this "soul food" may be good for your spirits, take a moment to think about what it's doing to your teeth.

How Could Something So Good Be So Bad?
Sugar can wreck havoc on your pearly whites. It's a complicated process, but here's the scoop. Bacteria in your mouth use your sweets as energy, growing and multiplying faster than they would otherwise. Some bond with the sugar to form a sticky glue called plaque. Plaque, in turn, produces acid. The acid dissolves the minerals that make your tooth enamel hard, and the surface becomes porous. The acid causes these tiny holes in the enamel to become bigger until one large hole appears. This is a cavity.

Does This Mean No More?
While ideally we would never expose our teeth to sugar or anything else that promotes tooth decay, that's just not realistic. All we can do is try to minimize the damage. Don't let your holiday routine interrupt your dental care regimen. Brush your teeth at least twice a day with a fluoride toothpaste (unless you're a young tyke), and floss at least once.

If you decide to indulge in more sweets than usual, it's a good idea to brush more often. If you don't have your toothbrush on hand, rinse your mouth out with warm water. Certain sugar-free gums can help as well. Choose those with Xylitol, an artificial sweetener that can help prevent tooth decay. And finally, try to mix up your snacks. If you're eating a sugary treat, try to also eat a bit of cheese (or a similar protein) as well.

Dr. Yam ran at the Philadelphia Marathon

Greetings!

We are very happy to announce that this past Sunday, November 21st, our very own Dr. Yam took part in the Philadelphia Marathon and finished among the top ten percent participants in the 26.2  mile run!


Dr. Yam  is an avid runner, biker and swimmer, who is next getting ready for the New Jersey marathon in April 2014, and the Iron Man Lake Placid in July 2014!

Excellent job,  Dr. Yam! We are so proud of you!

Some Dental Problems Are Easy to See!

One of the easiest dental problems to spot is a buildup of plaque. Plaque is the soft, sticky layer of bacteria that is constantly forming on the teeth. It is usually invisible to the naked eye. However, when a person is not brushing adequately, enough plaque can build up to appear as a thick, whitish coating on the teeth at the gum line. If not removed, it can lead to gum disease.

Another dental problem that is easy to spot is missing teeth. Many patients assume that if they are still able to eat, their teeth are fine. But often losing just one tooth can have a cascading effect on a patient's dental health. Losing one tooth can cause a loss of support; teeth may begin to drift into the empty space, causing a change in the bite. It also forces the remaining teeth to carry an additional load, sometimes past their ability to adapt. In most cases, when even one tooth is lost, the remaining teeth suffer and are more likely to be lost as well.

The Walk in Central Park


Greetings!


This past Sunday, October 20th, our office team was among the estimated 60,000 people who joined the American Cancer Society  for the walk in Central Park to support the fight against Breast Cancer.  It  was a great opportunity for us to gather outdoors and meet with so many people united by the idea of finding a cure!  This walk was a great success- it gathered  more people than any other previous walk in Central Park and as per the American Cancer Society's data it has already raised more than $2 million. We would like to take this opportunity and thank the ones who donated  to our team and who took the time to walk with us: Thank you, guys, without you it would not have been possible!

How to Pick the Right Dental Floss?

A lot of our patients admit that they do not floss because they find flossing uncomfortable and even painful, and when they see a little bloody residue on the thread they start to panic and this discourages them from continuing with this extremely important element of dental hygiene. We have to admit that brushing alone is not sufficient. Flossing does about 40% of the work required to remove sticky bacteria,  plaque, and food particles stuck between teeth, which otherwise will be left there if you only brush.  In other words don't feel you have completed your oral hygiene routine without flossing and rinsing with mouthwash.  If you fail do integrate all three components into your regimen, it is as if you  take a shower but forget to clean your ears or clip your nails!

It's amazing how much variety you can find in dental floss.  It is easy to find something that works well for you. All types remove cavity-causing plaque and food debris from between your teeth. Here are a few of your choices:
  • Dental Flossers: Often regarded as being easier to manage than conventional floss, flossers are a popular choice. There's no need to wrap any excess thread around your fingers, which some find uncomfortable.
  • Conventional Floss: There's nothing wrong with sticking with traditional types of floss. Just tear off a piece at least 18 inches long and wind it around your index fingers. If you find traditional thread-like floss is uncomfortable for your gums, try dental ribbon, its smoother counterpart.
  • Floss Threaders: If you have a fixed bridge in your mouth, it doesn't mean you get out of flossing. Nifty little floss threaders allow you to thread  the floss between your teeth, under the bridge work.

Sugar: The Scariest Halloween Trend

Halloween is approaching, and so are all those scary costumes and haunted houses. It shouldn't be the ghouls and ghosts that you fear, however, but the candied apples, trick-or-treat sweets and all the soda that washes them down! Sugary and acidic foods can lead to tooth decay, and that can put a damper on anyone's festive spirit.
 

While it would be nice if your neighbors all gave out fresh fruit or sugar-free candy (though we're sure the kids disagree!), high-sugar sweets are still America's favorite Halloween treat. If possible, try to limit children's sugar intake. Encourage them to drink plenty of water while snacking – soda will only make things worse! And finally, while it's easy to slip into a "sugar coma" and just pass out while watching a favorite horror flick, this is not the time to stray from your brushing or flossing routine. Let your children know that if they eat sweets without their toothbrushes on hand, they should rinse their mouths out with warm water.

Herald Square Dental Team Participates in the Walk Against Breast Cancer 2013 Campaign

October is the National Breast Cancer Awareness Month!  We are very proud to announce that, this year, our office is participating in the Walk against Breast Cancer in Central Park. The purpose of the walk is to bring awareness, fundraise and contribute to the fight against Breast Cancer.  It will take place on Sunday, October 20, 2013 at 8am starting at E72nd Street and 5th Avenue in Manhattan and we invite you to join our team. 

Why do we walk? 

It is unfortunate, but Breast Cancer has become so common that almost every one of us either has a family member or relative affected by it, or has lost someone close due to this illness.  We believe that, together, we are stronger and can find a cure! 

We encourage everyone to express their support for everybody affected - breast cancer survivors and those who could not beat the disease. By taking part in this event , we all have the chance to sponsor critical research in an effort to find a cure, raise awareness by encouraging and empowering women to take charge of their own breast health!  The American Cancer Society has been saving lives for 100 years. Today, they are helping save more than 400 lives a day that would have otherwise been lost to cancer. Are You Neglecting Your Dentures?

Old, uncared-for dentures are bad for your dental health and overall — and they also often contribute to bad breath.  We recommend that you have your dentures cleaned by a professional technician in our office at least once a year.

Although many of our patients hope that one denture will serve them a lifetime, dentures are not a lifetime investment.  ADA  recommends that dentures be replaced every 4-6 years.  If they are neglected or never replaced, they eventually develop a fungal smell and can be a home for plaque and bacteria, which contributes to developing halitosis (bad breath) and intricate a number of other health threats.  Make sure you clean your dentures at home every day and request a  professional denture cleaning in our office at your next check up visit.

Dr. Yam Raced in the IRONMAN Marathon at Mont-Tremblant, Canada!

Congratulations to our very own Dr. Yam!

On August 17th, Dr. Yam successfully completed his first Ironman race at Mont Tremblant- Quebec, Canada. The race consisted of  a 2.4 mile swim, 112 mile bike ride and a 26.2 mile run. From a total of 2,269 international participants, Dr. Yam crossed the finish line with a result of 12 hours 35 minutes and 18 seconds, which is better than the results of more than half of the participants! Great job, Dr. Yam! We are very proud of you!

Vitamin D Found Beneficial for Periodontal Health in Recent Study

Periodontal disease has established itself as a major culprit for oral health and lately more and more patients are diagnosed with this condition.  It starts presenting itself with the "harmless appearing" at first sight redness and inflammation of the gums. Even though these symptoms may appear harmless and not alarming, they are caused by the toxic waste products of oral bacteria and if left untreated may lead to tooth loss.

We would like to stress out the significant role which the immune system plays for oral health and how it could aid in the fight with periodontal disease and the possible reversal of  this condition. A good diet and in some cases, dietary supplements can enhance the immune system.Vitamin D might be helpful in controlling the inflammation associated with gingivitis and periodontitis. The upper limit recommended by the U.S. Food and Nutrition Board is 2,000IU. Researchers suggest this limit could be safely raised by about 10 times.
Researchers from India executed a three-month study which led to the conclusion that Vitamin D is a safe and effective anti-inflammatory supplement when given in a dose of 2,000IU per day which could aid in the prevention of periodontitis.

Don't forget your vitamin D supplement and spend time outdoors as much as possible since sun exposure is one of the most important sources of vitamin D!

Eating Disorders and Oral Health

You have heard it many times: the mouth is the window into the health of the body, right? Well, since our various cultures and lifestyles present to us the opportunity to gather around the table at every celebration and enjoy food in quantities much greater than the absolute needed for normal existing, and therefore we systematically consume larger amounts of food than needed, some people develop eating disorders.

According to the National Eating Disorders Association approximately  ten million Americans suffer from some type of an eating disorder.  Most frequently it is associated with either starvation ( in the cases of Anorexia- when one believes that they are "fat" when in reality they are not fat at all, ) or overeating and excessive vomiting to compensate the bulge eating, very often controlled by mood swings( in the cases of Bulimia- one would eat uncontrollably to satisfy the urge to eat, then feel guilty and vomit after a meal).

Eating disorders are frequently seen with teenagers and young adults, and more common among females.  The effect that they have on  the oral cavity, tissues and dentition is tremendous. The reason for it is the repetitive destructive and eroding effect of  stomach acids on tooth structures and the tissues in the mouth. With time, teeth become discolored, brittle, decay develops, and the tissues in the mouth become inflamed and bleed easily.  Eating hot or cold food or drink may become uncomfortable.

In many cases, the patient would feel ashamed discussing this issue and therefore fail to obtain professional help in a timely manner. However, it is extremely important to discuss with your healthcare provider the presence of such a problem if it exists. After all , even if you do not want to admit, a professional will be able to conclude the presence of such problem  by the obvious damage of the eating disorder.

If you suspect that a loved one is challenged by an eating disorder, don't be afraid to take the first step and initiate a friendly conversation to help them open up and acknowledge the fact that they need help if they want to overcome this problem. A timely intervention is important also because the damages caused may be irreversible, depending on the severity of the case and the length of time during which the problem was occurring.

Multiple Sclerosis and Oral Health

Multiple sclerosis is a progressive, neurological, and degenerative disorder which affects the central nervous system.  It affects about 400,000 people in the US alone and  usually more women than men are affected and diagnosed between 20 years and 40 years of age.
There are several symptoms of MS that can interfere with adequate care of the teeth and gums. These are distinctive and typical for the illness: fatigue, spasticity, weakness, tremors, facial pain, numbness, tingling, and/or pain in the hands, all of which can challenge a person’s effort to brush and floss adequately. Patiens feel absorbed and preoccupied  in their effort to control the symtpoms  of MS and fail to  maintain good oral habits. Therefore the tendency of poor dental hygiene in patients diagnosed with MS is becoming more prominent.

The following tips can compensate for these problems:
  • use toothbrushes with built-up handles(or cut a small slit in the sides of a tennis ball and slide it onto the handle of the toothbrush);
  • use flossing tools; consider electric toothbrushes and flossing devices;
  • sit to brush and floss, if standing at the basin is tiring; 
  • floss in the morning if you are too tired at night; 
  • allow a family member or personal assistant to help with tooth brushing and flossing;
  • manage tremors by wearing a weighted glove while brushing.
Since some medications used to treat MS symptoms can cause dry mouth, it could be especially challenging  for affected patients to maintain proper dental health.
Here is how you can minimize the effects of dry mouth:

  •  sip water or sugarless drinks often;
  •  avoid caffeine, tobacco, and alcohol;
  •  use a small squirt of lemon in the mouth or sugarless lemon candies to stimulate the glands controlling saliva production;
  •  use a humidifier at night;
  •  use special products that are available in pharmacies to moisten dry mouth!

Severe Gum Disease Can Instigate Joint Replacement Infections

We thought you should know: a thorough dental examination for presence of tooth and gum infection is recommended prior to joint replacement.

Periodontal disease is a commonly occurring oral infection of the adult population (80% of adults are affected) destroying the bony support of the teeth. This infection slowly progresses over the lifetime of the patient and is often manifested in the advanced stage in the senior population.

Because of the lack of any discomfort to the patient until total loss of bony support to the teeth, many adults are unaware that they have this infection. However, the bacteria from this bone destroying disease, especially in the more advanced cases, has been attributed to possible infection of joint replacements.

Candidates for joint replacements who have not been seen by a dentist in over a year should be considered for a thorough dental evaluation of their teeth and gums before surgery. Depending on the severity of the infection, treatment of gum infections and tooth abscesses may take from two weeks to two months before the patient reaches adequate health before surgery.

Dental Decay & Sealants

Fluoride has been a great benefit to patients of all ages in helping prevent dental decay. Regular brushing and flossing lowers the chance of developing cavities. However, sometimes good habits alone aren't enough. The most decay-prone areas of teeth are the grooves and depressions on the chewing surfaces of the back teeth. These vulnerable areas require further preventive care.

To protect kids' teeth from decay, a plastic-like coating called a sealant should be painted on the chewing surfaces of all the back teeth. It's a simple process that's extremely effective: studies have shown that sealants can reduce tooth decay by as much as 90% to 100%.

The American Dental Association recommends sealants be placed as soon as the first adult back teeth come in at age 6 or 7. Cleaning My Baby's Teeth

Cleaning you infant's teeth is a habit you should definitely start early! The teeth must be cleaned as they erupt. Use a damp wash cloth or a soft rubber sleeve with toothbrush-like bristles to gently massage gums and the newly erupted baby teeth.

If your pediatrician agrees, use a tiny dab of fluoride toothpaste for infants. Tooth brushing is definitely a parent's job during the preschool years as well. You should always monitor the brushing process to minimize toothpaste swallowing and to ensure proper oral hygiene has been performed.

Children are usually able to brush their teeth well when they are 8 years old. Be sure to check your child's teeth regularly for any chalky white or brown spots which could be the beginning of tooth decay.

Diet and Dental Health

Diets low in certain nutrients reduce resistance to oral and dental infections, that is, periodontal disease also known as gum disease, and tooth decay. A healthy immune system is essential to controlling periodontal disease.

Healthy and nutritional diet will improve dental health and overall health. The consumption of sugar, especially in sticky forms or in a baby bottle while sleeping, contributes to the rapid development of dental decay.

The trace nutrient fluoride, may not be adequately supplied by bottle or municipal water supplies. Supplementation with oral tablets and topical application will reduce the incidence of dental decay by more than 60%. Or, as an alternative please remember to brush with flouridated toothpaste and choose tap water over bottled water, as the tap is surely flouridated whereas tap water usually is not.

Together, a balanced diet, daily use of fluoride, effective brushing and flossing, and sensible eating habits, along with regular dental check ups and preventative visits can reduce the risk of, or even prevent, infectious dental disease.

Why Should You Floss?


The purpose of flossing, as well as brushing, is to reduce the number of bacteria which inhabit our mouths. Flossing removes the bacteria that escape the toothbrush by hiding in the tiny spaces in between teeth.

Floss is the only thing that can really get into that space between the teeth and remove bacteria. If you only brush your teeth and skip the flossing element, consider that you have done a half way job cleaning your teeth! You may wonder why? Think of it this way: each tooth has five surfaces; if you don't floss, you are leaving at least two of the surfaces unclean( usually the ones that are in contact with the adjacent teeth). That  is a great place for bacteria and plaque to settle and for a cavity or a periodontal pocket to form.

Most floss is made of  nylon. People with larger spaces between their teeth or with gum recession tend to get better results with a flat, wide dental tape. If your teeth are close together, try thin floss. In any event you should make flossing a part of your regime and try not to skip it. By brushing and flossing, we help eliminate the bacteria which causes bad breath, gingivitis, and periodontal disease thus creating healthy smiles which last a lifetime!

Bruxism: Not the Usual Grind

Do you think you grind while sleeping? Sure, it's annoying when friends, spouses or children grind their teeth at night, but the effects could be much more serious than the loss of a good night's sleep on your part. This sleep disorder, called bruxism, can lead to a number of ailments, including loose or lost teeth. Even lighter grinding can wear down tooth enamel. Bruxism can also cause another condition called temporomandibular joint disease (TMJ). TMJ occurs when the muscles, joints and ligaments of the jaw move out of alignment. Symptoms of TMJ include headache, earache and pain in the face, neck or shoulder.

"But," you ask, "don't we all grind our teeth once and a while?" It's true that a number of people grind their teeth during periods of stress, but chronic grinders – those that suffer from bruxism at least once a week – could eventually pay the price. Not only can it lead to the ailments already mentioned, but can result in daytime sleepiness and headaches as well.

Research has also discovered that people who brux are more likely to snore, suffer from breathing pauses during sleep and be victims of sleep apnea. As is the case in many sleeping problems, stress is a major cause. It's not unusual to clench or grind the teeth when angry or nervous. The use of tobacco, alcohol or caffeine tends to aggravate the problem. What's more, nobody is immune. All age groups and genders are susceptible to bruxism, though most cases occur among those between the ages of 19 to 44.

There are a number of things you can do if you feel either you or somebody you love is suffering from bruxism. For starters, try to alleviate whatever stress in your life you can. Kick the tobacco habit and heavy drinking, and try not to drink too much coffee. Severe cases may need to be fitted with a mouth guard or have their teeth reshaped to eliminate unusual alignment. Are You Interested in Dental Implants?

When it comes to implant treatment, there may be a few possibilities and  variations in the procedures involved. The process begins with a thorough evaluation of the patient's medical and dental history, and a clinical examination of the entire mouth and missing tooth area, study of your jaw bone anatomy and density. The clinical exam may also include specific X-rays, in some cases a CT scan might be needed. All of that could be done at the consultation visit in our office, so there is no need for you to go to another practice for additional visit and waste time!  We understand your time is valuable and therefore work hard to minimize the lenght of the entire treatment experience, so that you can resume normal lifestyle as soon as possible. 

Once the dentist has reviewed the data from the CT scan, he will be able to propose a more comprehensive and precise treatment plan. Our treatment coordinator will prepare the quote for your treatment cost and work with you on the financial arrangements. 
The surgery date will be scheduled next. At that visit  implants are surgically placed in the jawbone under local anesthesia, if extractions and bone graft are a part pf your treatment, they will be performed at this visit as well. You will be allowed some healing time and requested to return for a post op visit in about a week or so following the implant placement surgery. The length of the healing time is based on the quality and quantity of bone, as well as the type and number of implants placed, the type and number of extractions done. After adequate healing time is allowed, the implant will be used to support a partial, denture or restored with  the help of a crown or bridge.

Sugarless Gum and Xylitol Help Fight Cavities!

We all  love chewing gum, the proof is all over our streets- we chew gum and after it looses its taste we spit it on the street ( well, maybe not all of us do that, but a great number unfortunately do! ).  However, not just any gum being sold in the store is actually helping us in our fight against cavities. 

 The American Dental  Association ( ADA) has created a list of products that have met its requirements to be awarded with the ADA Seal of Acceptance, which is an indicator that the product owning it has met the requirements needed and the manufacturer has accordingly presented the corresponding data from clinical or laboratory studies that support the product's safety, effectiveness and promotional claims  ot in other words that it basically does what is advertised on the packaging.

The products in the group of the chewing gum contain Xylitol which reduces decay-causing bacteria.  By using Xylitol products throughout the day, bacteria is prevented from adhering to the enamel and creating the acids that damage the teeth. Xylitol enhances the remineralization of teeth, particularly in small decay spots just developing in the tooth enamel, normalize the  pH levels, prevents demineralization.  Xylitol's naturally cooling and sweet tastes also increase saliva production which is another way to fight the growth of cavity causing bacteria.

Asians Are at Greater Risk of Periodontal Disease?

A recent study of the clinicians from the University of California in San Francisco (UCSF) suggested that the teeth roots are significantly shorter in people with Asian origin, which automatically predisposes them to higher risk of developing periodontal disease. Previously, the reason given for the higher incidence of periodontal involvement in Asians was attributed to poor hygiene, however this study rules this out as the cause and confirms that Asians are genetically predisposed to develop periodontal disease. Regular preventative care in these patients  is of ultimate importance to maintaine healthy gums. Lost Teeth Impact Your Overall Health

In the past, the loss of teeth was dismissed as a natural part of the aging process, and dentures were considered a normal sign of old age.

While such ignorance is rapidly becoming a thing of the past, there is still not a general awareness of the seriousness of edentulism (the condition of being completely toothless). Multiple tooth loss is most commonly caused by periodontal disease, i.e.. gum and bone disease. Periodontal disease has a high statistical correlation with chronic debilitating disease, such as diabetes, cardiovascular disease and arthritis.
So it is not very surprising that studies now show a shortened life expectancy by as much as 10 years for persons who have lost their natural teeth, when compared to those who have kept their teeth.

In addition to longevity, quality-of-life differences are just as significant. The functional efficiency of dentures has been measured to be only 20% of that of natural teeth. Can there be any doubt that nutritional deficiencies will be much more prevalent in those without their natural teeth?

Cranberry Juice Can Help Prevent Cavities!

We already know that cranberries are packed with powerful antioxidants that have been proven to fight bladder infections, and now they can help us fight cavities! According to a scientific research team led by oral biologist Hyun (Michel) Koo, D.D.S., Ph.D from the University of Rochester Medical Center, cranberry juice covers the tooth enamel with a shield, just like "Teflon  for teeth", which makes it difficult  for bacteria to adhere to the tooth surface and hence prevents formation of plaque and the development of the Streptococcus mutans which create most cavities ( by eating sugar and then excreting  acids  that cause decay).  The important thing to remember that the full benefit of the cranberry juice can only be obtained when it has no added sugar(contrary to the expectations, most common brands on the market are packed with added sugar), therefore remember to read the ingredients list  on the products you purchase at all times and indulge in this yummy drink knowing that it is actually helping you keep healthy teeth!

Does Being a Vegetarian Affect Your Dental Health?


Of course being a vegetarion is a matter of personal choice and we accept it as perfectly normal. However, we would like to make sure that you, as our patients, are informed on special preventative measures  and extra care tips you should implement to ensure you are taking the best care possible for your body and oral health.

Most adult vegetarians know the importance of balanced nutrition and therefore carefully monitor their diet. The potential for deficiencies is greatest among children and teenagers, and young adults who put themselves on vegetarian/ limited diets without knowing enough about their nutritional needs. Be sure to take the time and educatie your children and loved ones on the importance of balanced and nutritional diet, and the need of variety  in their menu.

Vegetarians could be a subject to a limited nutritional, mineral  and vitamin intake and therefore can experience deficiencies in calcium, vitamin D, riboflavin, vitamin B12 or complete proteins. This can put them at greater risk for tooth decay, enamel erosion and gum disease, and other health risks apart from the ones in the oral cavity. Studies are contradicting on whether by eating the right amount of fruits, vegetables, grains and leafy greens, vegetarians can consume the nutrients they need. Therefore additional vitamin supplement is strongly  recommended.

Vegetarian diets and diets in which fruit comprises more than two-thirds of the total food intake also make individuals more susceptible to dental erosion. Not having enough of the everyday essential nutrients might soften teeth, leading to decay and possibly periodontal disease. Vegetarians might not be able to absorb calcium as well as other people with different diets. Frequent vomiting and acid reflux, both of which can introduce stomach acid into the mouth, also can cause dental erosion.

Oral Health for Seniors

The good habits of effective daily brushing, diet, and use of fluoride will help the aging person adapt to changing conditions. Reduced salivary flow and addition of medications will affect oral health dramatically.

Changes of the teeth and gums: Teeth will darken because of long-term exposure to plaque and changes in the dentin within the tooth. The gums may recede and uncover the roots. Exposed roots will be darker than the enamel part of the tooth and are prone to decay. Fillings can decay and darken, too, as they weaken over time.

"Dry Mouth" may develop: Saliva is useful to lubricate, wash away plaque, and neutralize the acid produced by plaque. Flow of saliva can be reduced by a medical disorder or be a side effect of antihistamine, decongestants, pain killers or diuretics.

The build-up of plaque will accelerate tooth decay. There will also be more gum infection which will cause a loss of bone support for the teeth. Poorly-fitting or poorly-cleaned dentures, illness, and some medications increase the severity of the problem.
